Do I need to bring anything with me to check in?

Yes, Please bring a photo ID and your insurance card(s).

Why do I need to provide a picture ID and my insurance card every time I visit?

Centegra needs to verify your personal and insurance information each visit to protect your identity.

I gave my insurance to Centegra Hospital. Why don’t you have it?

Centegra Hospitals have a separate billing system than the physician practice. In addition, your insurance benefit coverage may be different for hospital services than it is for physician services.

Do I need to pay my co-pay or deductible at the time of service?

We require your coinsurance, co-payment and/or deductible to be paid at the time of service. We accept cash, check, money orders and credit cards.

How do I contact Centegra Physician Care Patient Business Services about my bill?

What are the ways to pay my bill?

Centegra Physician Care has makes every effort to make paying your bill an easy process.  There are three ways to pay.

Insurance Questions

 

Will Centegra Physician Care bill my insurance?

Yes, please bring your insurance card on the day of your service. You will also need to bring along any insurance forms your insurance company requires such as accident form, claim form, coordination of benefit form or a pre-existing condition form. Please bring any other health forms you think may be needed.

If I don’t have insurance coverage, will I be seen?

Centegra Physician Care will not withhold or delay emergency services because of a patient’s inability to pay. Centegra offers a variety of payment alternatives such as interest-free payment plans.  A $50 deposit toward the charges for that day's visit is required.  All major credit cards are accepted.
